This resource page provides several documents for you to view and download if you wish. New patients to the Grantham Denture Clinic are requested to fill in a couple of forms before the first procedure your denturist provides (except for the Free Consultation). Normally you are given a paper form in-person to fill out in the office at that time. However, if you wish you can download those forms below, print them out and fill them in at your leisure at home instead, and bring them with you to your appointment. In addition, if you are getting New Denture(s) made, at the end of your final appointment to Insert them, your denturist will explain to you how to take care of your New Dentures and other special instructions, and give you a print-out on paper of those same instructions. Patient Registration Form
New patients to the Grantham Denture Clinic are requested to fill in a Patient Registration Form before the first procedure your denturist provides (except for the Free Consultation). This only needs to be done once. Normally you are given a paper form in-person to fill out in the office at that time. Some people prefer to fill it in at home and bring it to the office at their first procedure appointment. Or perhaps a relative or friend will help you fill it out. So you or they can view, print or even download it to your computer here.
Patient Consent Form
New patients to the Grantham Denture Clinic are requested to sign a Patient Consent Form, giving us permission to work on you, before the first procedure your denturist provides (except for the Free Consultation). This only needs to be done once. Normally you are given a paper form in-person to sign in the office at that time. But if you would like to read it at your leisure at home, or even print it out and sign it at home, or download it, you can do so here.
Home Care Instructions

Instructions for Downloading a PDF Document
If you are actually downloading the document (instead of just viewing it or printing it from this page), then note that the document file is in PDF format.
